In collaboration with Northwest School District, Compass Health Network has a location on the campus of Valley Middle School where students can receive dental and mental health services.

Services are available to students and their families Monday-Friday from 7:30am-6:00pm, and available to the community Monday-Friday from 2:30pm-6:00pm. Compass accepts most insurance plans; Medicaid, Medicare and self-pay. An income -based sliding fee scale is offered to reduce costs for eligible applicants. Additionally, Compass offers assistance with navigating Medicaid applications.
Student transportation between the Northwest district schools and Compass Health Network (Valley Middle School location) is available during school hours by a district staffed liaison. Written consent is required for services and transportation. (See transportation form above.)
In addition to services at the Valley Middle School location, the Compass Tooth Titans mobile program periodically visits the district school campuses to provide on-site dental services, which increases accessibility and serves more students.

Tooth Titans mobile program offers two options for families:
Dental screening including fluoride treatment/sealants and dental education
Dental Exam includes the above, plus, comprehensive exam by dentist and x-rays, teeth cleaning - Insurance billed.

Compass is a Federally Qualified Health Center and a Certified Community Behavioral Health Organization with 20 locations across Jefferson County. If you don't have insurance, a sliding fee scale is available, contact the Compass Central scheduling for assistance. 877-266-8732
